I plan on writing forced to do it for work on my consent form. I doubt they will care that it is against valid consent as you experienced. Then if there is ever a review years from now it’s recorded.
There is not physical consent forms here, just verbal.

I did raise the valid consent thing earlier and even emailed the Premier and health minister as to how one were to give valid consent given their mandates are in place. It would technically void all consent as you wouldn’t be able to tell if a person did it willingly or due to the coercion of the mandate. Obviously crickets.
I wonder if signing a form is dependent to the vax center or a state by state thing. The missus went and got a new script for the pill, they wanted to vax here at the same time but she declined so they gave here a vaccine consent form to fil out for if she gets the jab.

I went to shoot at my local pistol club on Sunday, and as I approached the ‘coloreds only’ entrance doors to enter and sign my name in crayon as I usually do, I was stopped by a LITERAL NAZZZEEE at the door.
“PAPIERS PLEASE” he said, in an obvious reichsvolk accent. I showed him my Firearms licence. “NEIN. ZEEZ PAPERS ARE VERBOTEN EIN VILL NOT BE ACCEPTED” he said loudly. “VERE ARE YOUR DOUBLE VAXX PAPIERS, YOU BIG SCHMELLY SCHWAZERMAN?”
I replied in a chocalatey voice “I don’t gots none. Im a pure-blood ya dig?” my footlong member showing through my skin tight leather jeans. Or maybe it was a zucchini I forgot about.
The whole club erupted in laughter and began pointing at me. “HE’S NOT VAXXED!”
“HA HA”. “HA HA”. said a fat kid repeatedly as he pointed.
“GET A LOAD OF ZIS UNVAXXED FOOL JA?” said another as he slurped warm piss from a jug.

Spiritbroken, I turned around and did the long walk of shame back to my '72 Camaro Iroc Z and drove out of that hellhole.
