Different name, it’s a OTs-62 12 Gauge “Service Revolver Shotgun” but it is derived from the MTs-225.
Other than the grip I think it looks pretty cool.

That’s correct. Made by the Polish resistance in several workshops then assembled centrally. Figures vary but around 150-250 made. Some parts were made as hospital bed legs, others as vacuum cleaner parts in Nazi controlled factories.
